On 13 August 1967, Staff Sergeant Alvin Ray Bozarth was serving in E Company, 4th Battalion, 12th Infantry, 199th Infantry Brigade in Binh Duong Province, South Vietnam.

On that day, Staff Sergeant Bozarth was killed in action when the helicopter in which he was riding crashed. His body was recovered.
