On 28 April 1969, Lieutenant (Junior Grade) Joseph Felder Hart was serving with HA(L)-3, TF 116, U.S. Naval Forces Vietnam, in Cambodia.

On that day, LTJG Hart was killed in action in the crash of the helicopter (due to hostile action) in which he was a pilot. His body was recovered.
