On 14 May 1970, Sergeant Donald Gary Kuzilla was serving with A Company, 4th Battalion, 31st Infantry Regiment, 196th Infantry Brigade, in Quang Tin Province, South Vietnam.

On that day, SGT Kuzilla was killed in action after sustaining multiple fragmentation wounds. His body was recovered.
