On 23 March 1969, Staff Sergeant Rex Fillmore Church was serving with Headquarters & Headquarters Battery, 1st Battalion, 5th Artillery, II Field Force, in Bien Hoa Province, South Vietnam.

On that day, Staff Sergeant Church was killed in a non-hostile vehicle crash. His body was recovered.
