On 12 November 1970, Sergeant Donald Warren Chipp, Jr. was serving with B Company, 3rd Battalion, 1st Infantry, 11th Infantry Brigade, Americal Division, in Quang Ngai Province, South Vietnam.

On that day, Sergeant Chipp was killed in action when he sustained wounds from an explosive device. His body was recovered.
