On 5 May 1968, Sergeant Garry Raymond Chapman was serving with E Company, 2nd Battalion, 39th Infantry Regiment, 9th Infantry Division, in Dinh Tuong Province, South Vietnam.
On that day, Sergeant Chapman was killed in action when he sustained multiple fragmentation wounds. His body was recovered.
