On 1 June 1970, Lieutenant John Martin Mulcahy was serving with HA(L)-3, TF 116, U.S. Naval Forces Vietnam, in Phong Dinh Province, South Vietnam.

On that day, LT Mulcahy died of other causes in the crash of the helicopter (due to non-hostile action) in which he was a pilot. His body was recovered.
