On 14 December 1969, Petty Officer First Class Larry Richard Johnson was serving with HA(L)-3, TF 116, U.S. Naval Forces Vietnam, in Gia Dinh Province, South Vietnam.

On that day, PO1 Johnson was killed in action in the crash of the helicopter (due to hostile action) in which he was a crew member. His body was recovered.
