On 23 April 1969, Petty Officer First Class Ollie James Gross was serving with DET-4, HA(L)-3, U.S. Naval Forces Vietnam, in Hau Nghia Province, South Vietnam.

On that day, PO1 Gross died of other causes in the crash of the helicopter (due to non-hostile action) in which he was a crew member. His body was recovered.
