On 13 May 1968, Sergeant Joseph Adams was serving with Provisional Signal Company, 125th Signal Battalion, 25th Infantry Division, in Tay Ninh Province, South Vietnam.

On that day, SGT Adams was killed in action when he sustained wounds from artillery, rocket or mortar fire. His body was recovered.
