On 4 September 1968, Sergeant Thomas Alan Ackerman was serving with B Company, 3rd Battalion, 12th Infantry, 4th Infantry Division, in Kontum Province, South Vietnam.

On that day, SGT Ackerman was killed in action when he sustained wounds from small arms fire. His body was recovered.
