On 27 October 1968, Warrant Officer 1 Dennis Carlton Ackerman was serving with the 92nd Assault Helicopter Company, 10th Aviation Battalion, 17th Aviation Group, 1st Aviation Brigade, in Tuyen Duc Province, South Vietnam.

On that day, WO1 Ackerman was killed in the crash of the helicopter he was flying after it was shot down. His body was recovered.
