Corporal Forman was a member of the 5th Ordnance Medium Maintenance Company, U.S. Army. On 17 October 1950, he was riding in a truck near Chindong, South Korea, when the truck overturned killing him.

Forman was awarded the Korean Service Medal, the United Nations Service Medal, the National Defense Service Medal and the Republic of Korea War Service Medal.
