On 21 February 1970, Sergeant Jeffrey Thomas Beardsley was serving with C Battery, 4th Battalion, 42nd Artillery, 4th Infantry Division, in Binh Dinh Province, South Vietnam.

On that day, SGT Beardsley was killed in action when he sustained wounds from Friendly Fire. His body was recovered. He received a posthumous promotion to Staff Sergeant.
