On 25 February 1966, Private First Class Phillip Anthony Lotta was serving with G Company, 2nd Battalion, 9th Marines, 3rd Marine Division, in Quang Nam Province, South Vietnam.

On that day, PFC Lotta was killed in action by misadventure. His body was recovered.
