In 1918, Theodore H. Campbell, Jr. was living in Morris Plains, New Jersey, where his last known civilian occupation was serving as an attendant at the New Jersey State Hospital (also known as Greystone).
Campbell enlisted in the Army on 2 April 1918 and by 20 May 1918 he was serving overseas in World War I with Unit 15 Co 153 Depot Brigade. He later served in Company M, 310th Infantry Regiment, 78th Infantry Division which, on 16 October 1918, began fighting in phase one of the Meuse-Argonne Offensive. Private First Class Campbell was wounded in action on 22 October 1918 and died of his wounds the same day.

Private First Class Theodore H. Campbell, Jr. is buried at the Meuse-Argonne American Cemetery and Memorial, Romagne-sous-Montfaucon, Departement de la Meuse, Lorraine, France.
