Donald Wayne Hart was born on 12 September 1945.

On 31 July 1967, Specialist Four Donald Wayne Hart was serving with 189th Assault Helicopter Company, 52nd Aviation Battalion, 17th Aviation Group, 1st Aviation Brigade, in Pleiku Province, South Vietnam.

On that day, SP4 Hart was killed while serving as door gunner in a helicopter that flew into IFR conditions while flying close to the ground. In the fog and mist, the helicopter ran into a tree and crashed, killing the entire crew. His body was recovered.
