John Ralph Frongillo was born on 26 January 1949.

On 15 May 1969, Warrant Officer John Ralph Frongillo was serving with B Company, 1st Squadron, 9th Cavalry Regiment, 1st Cavalry Division, in Binh Long Province, South Vietnam.

On that day, WO Frongillo sustained injuries when the helicopter in which he was a crew member was shot down. WO Frongillo died of his injuries on 17 May 1969.
