James Warren Donnelly, Jr. was born on 1 March 1948, the son of James Warren and Mary Willa House Donnelly.

On 5 August 1969, Staff Sergeant James Warren Donnelly, Jr. was serving with B Company, 4th Battalion, 9th Infantry, 25th Infantry Division, in Binh Duong Province, South Vietnam.

On that day, SSGT Donnelly was killed in action when he sustained multiple fragmentation wounds. His body was recovered.
