James T. Donahue, Jr. was born on 12 February 1945.

On 22 September 1967, Warrant Officer James T. Donahue, Jr. was serving with 188th Assault Helicopter Company, 269th Combat Assault Battalion, 12th Combat Assault Group, 1st Aviation Brigade, in Hua Nghia Province, South Vietnam.

On that day, WO Donahue's helicopter was shot down during a combat assault and, despite receiving immediate medical care, he did not survive. His body was recovered.
